Directions to Complete AYSO Volunteer Application

Before starting, have the following things ready:

      • email address unique to volunteer
      • last 4 digits of volunteer’s SS#
      • photo ID (driver’s license, passport, etc)
      • name, phone #, address for two personal references.

Note: If you volunteered earlier than 2016, and this is your first time in Blue Sombrero/Sports Connect, it’s important you use the same First Name, Last Name, DOB, and last 4 digits of SS# now as you did in eAYSO, in order to inherit your AYSO ID# and certification history from eAYSO.

1 – Go to Sports Connect’s Region 13 home page: http://ayso.bluesombrero.com/region13

2 – Create or login to your account. This account should be tied to an email address unique to the volunteer. If you are registering a youth volunteer (under 18), the minor should have his or her own email address..

3 – Return to Sports Connect’s Region 13 home page: http://ayso.bluesombrero.com/region13

4 – Click on the tab called “Volunteer Applications.” Find the volunteer application for the year for which you are volunteering, then click “Sign Up“.

Note – Please be careful when selecting the year. Between April 1 and July 31 of any year, multiple years may be active at the same time.

5 – On the next page, click on the green ‘Sign Up’ button next to the appropriate title, then scroll to the bottom and click Continue.

6 – On the next page, select your name/icon, then scroll down and click “Continue” again.

7 – Now you are at the actual application form. Fill it in.

TIP: Answer only required fields (marked with a red asterisk *). If you provide optional answers, the page might reload – harmless, but confusing and time-wasting.

TIP: In the box labeled “Reference #1 Organization Name” – you can enter an employer or other organization where you have held a position. For returning volunteers, this can be AYSO. On the next line below this answer, enter the position you held.

TIP: You’ll see a text box with the confusingly similar label “Reference #1 First Name.” Here you should enter the name of a personal or professional reference. This person does need to have any affiliation with the organization you listed above.

8 – Accept Waiver: Click on it to make the answer turn to “Yes.”

Before submitting your application form, please double‐check all of your answers. Submitting incorrect answers will cause a bunch of headaches later.

9 – Click “Submit,” then follow the steps to provide your “E-Signature.” Don’t worry if some of the answers from the questionnaire don’t appear on the printed version of the form.

10 – When asked, consent to a Background Check (“Volunteer Verication”).
This is important, or else the process will not be complete.

11 – Follow prompts all the way thru to click “Submit.”

12 – Finally, you’ll see a confirmation of your submission that looks like this:

13 – Look for an e-mail from Verified Volunteers or Sterling, and make sure to follow its prompts to initiate the required background check.

With that, you’re done!
