Coaches or team managers may use this online form to submit required information for their team referees. Practice field assignments and/or team uniforms will not be released until the contact information for a minimum of 2 referee volunteers is supplied and confirmed. The coach may not be one of the 2 required referees. Please advise your referee volunteers of the referee class schedule that is available via the Region 13 website for applicable training requirements.
Note that you will only be able to submit this form one time. Please make sure that the information is complete and accurate. If changes and/or additions need to be made to previously submitted team referee information, please contact your Division Referee Coordinator. The names and contact information of all Division Referee Coordinators can be found on the Referee Staff page.
